About
The Autism Outreach Team is a government service operated by States of Guernsey Health & Social Care within the Adult Disability Service. Based at Professor Shaw Community Centre in St Martin, the team supports adults with autism to access community activities and live independently, working with individuals in family homes, private tenancies, and extra care housing. Services include person-centred care planning, multi-disciplinary coordination, and community inclusion support. Referrals are made through the Adult Resource Allocation Panel. —
