About

LD Outreach Team is a States of Guernsey Health & Social Care service supporting adults over 18 with learning disabilities to live independently in the community. Operating from the Professor Shaw Community Centre in St Martin, the team uses an Active Support Model to provide person-centred support in private tenancies, Guernsey Housing Association properties, and extra care housing, covering personal care, medication, daily living skills, and community access. Referrals are accepted via GP, family, health professionals, or self-referral through the Adult Resource Allocation Panel.
